Two direct hire opportunities are available for inpatient pharmacists specializing in oncology and hematology. The positions require working in an acute care inpatient setting with a focus on malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) patients. These roles offer a typical inpatient schedule with rotating days, evenings, and weekends.
Responsibilities:
- Participate in interdisciplinary rounds daily to assist with medication regimen design, implementation, and monitoring.
- Review drug therapy for appropriateness, considering dosage, interactions, allergies, and cost-effectiveness.
- Reconcile home medications on patient admission.
- Clarify medication orders with healthcare providers and document changes in patient records.
- Counsel patients and families on medication regimens, adherence, and adverse effects before discharge.
- Provide pharmacokinetic consultations and document notes when required.
- Monitor and report adverse drug reactions and medication errors.
- Review chemotherapy orders for assigned patient groups.
- Support nursing staff with medication order questions.
- Verify orders and manage in-basket messages for assigned units.
- Staff afternoon shifts as needed.
- Facilitate appropriate medication route changes and assist with chemotherapy desensitizations.
Qualifications:
- Bachelor of Science in Pharmacy.
- Illinois pharmacist license or eligibility to obtain one.
- Postgraduate ASHP-accredited PGY2 residency in Hematology/Oncology preferred.
- Board Certification in Oncology Pharmacy (BCOP) required within 18 months of hire.
- BLS Certification.
- Experience with intravenous admixtures, drug therapy, chemotherapy, and cellular therapies.
- Strong teamwork, problem-solving skills, and ability to work independently under pressure.
- Candidates with solid oncology experience but without residency training are also encouraged to apply.
