Two Direct Hire Opportunities for Oncology/Hematology Inpatient Pharmacists

Immediate openings are available for two inpatient pharmacists specializing in oncology and hematology to join a leading medical center in Chicago, IL. Positions involve supporting hematology, oncology, gynecologic oncology, and stem cell transplant (SCT) patient care teams. Applicants should be ready to start as soon as possible and open to obtaining an Illinois pharmacist license if not currently licensed.

Schedule & Location:

  • Typical inpatient pharmacy schedule with days and evenings
  • Rotating weekends
  • Based in Chicago, IL

Responsibilities:

  • Participate in daily interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology, and SCT patient care
  • Review and verify chemotherapy orders and other medication regimens for safety, efficacy, and cost-effectiveness
  • Reconcile home medications upon admission
  • Document medication order changes and counsel patients and caregivers on medication adherence and side effects
  • Provide pharmacokinetic consults, including vancomycin and aminoglycoside dosing adjustments
  • Detect, document, and report adverse drug reactions and medication errors
  • Support nursing staff with medication order questions and conduct order verification activities
  • Assist with medication transitions from IV to oral routes and chemotherapy desensitizations as needed
  • Participate in PM staffing based on departmental requirements

Qualifications:

  • Bachelor of Science in Pharmacy
  • Illinois pharmacist license or eligibility to obtain licensure
  • Postgraduate Year Two (PGY2) ASHP-accredited Hematology/Oncology residency highly preferred, but candidates with solid oncology experience will be considered
  • Board Certification in Oncology Pharmacy (BCOP) preferred before or within 18 months of hire
  • Current BLS certification
  • Strong knowledge of intravenous admixture, drug therapy, chemotherapy, and cellular therapies
  • Excellent problem-solving skills with ability to work independently and collaboratively
