Two Direct Hire Opportunities for Inpatient Oncology/Hematology Pharmacists – Chicago, IL
Seeking dedicated inpatient pharmacists with a solid oncology/hematology background to join a leading medical center in Chicago. These full-time roles require strong clinical expertise in acute care oncology and hematology, providing critical pharmaceutical support to interdisciplinary care teams. Schedule: Typical inpatient schedule with days/evenings and rotating weekends
EMR: EPIC system used
Key Responsibilities:
- Participate in interdisciplinary rounds focused on malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant patients.
- Review and optimize drug therapies for appropriateness, dosing, interactions, and cost effectiveness.
- Reconcile home medications upon patient admission.
- Clarify medication orders with healthcare teams and document changes in patient records.
- Provide medication counseling and adherence support to patients and their families, particularly for transplant cases.
- Perform pharmacokinetic consults, especially for vancomycin and aminoglycosides.
- Monitor, document, and report adverse drug reactions and medication errors.
- Review and verify chemotherapy orders for inpatient oncology and hematology patients.
- Support nursing staff with medication-related inquiries and order verification for assigned units.
- Assist in chemotherapy desensitization procedures in intensive care units as needed.
- Provide evening and weekend coverage based on department requirements.
Desired Qualifications:
- Bachelor of Science in Pharmacy
- Illinois Pharmacist License or eligibility to obtain license
- Postgraduate ASHP PGY2 Pharmacy Residency in Hematology/Oncology preferred
- Board Certification in Oncology Pharmacy (BCOP) prior to or within 18 months of hire preferred
- BLS Certification
- Solid knowledge of intravenous admixture, intravenous drug therapy, chemotherapy, and cellular therapies
- Strong collaboration skills, problem-solving ability, and capacity to work independently under pressure
