Pharmacist in Chula Vista, CA

A remote opportunity is available for a California-licensed Pharmacist to participate in a Medication Therapy Management (MTM) pilot program. This part-time contract role requires residency in California and enrollment in Medicaid as an Ordering, Referring, and Prescribing (ORP) provider. The position offers flexible scheduling with 10-20 hours per week, primarily during day shift hours, to support patient outreach, counseling, and clinical pharmacy initiatives.

Location: Remote within California (CA resident required)

Schedule:

• Approximately 10-20 hours per week
• Flexible hours, primarily day shift for patient counseling
• Guaranteed minimum of 8 paid hours weekly regardless of workload

Responsibilities:

• Conduct MTM consultations to optimize medication therapy
• Provide patient counseling and education on medications
• Review medication profiles and identify optimization opportunities
• Accurately document clinical interventions and patient encounters
• Collaborate with program stakeholders to improve patient outcomes
• Ensure compliance with state and federal regulations

Required Qualifications:

• Active, unrestricted California Pharmacist license
• Current residency within California
• Prior experience in Medication Therapy Management
• Enrollment in California Medicaid (Medi-Cal) as an ORP provider
• Strong patient counseling and communication skills
• Ability to work independently in a remote setting with flexible scheduling capabilities
