A California-licensed Pharmacist is sought for a remote Medication Therapy Management (MTM) pilot program. This part-time contract offers flexible scheduling with 10-20 hours per week, allowing you to work remotely anywhere within California. The role supports MTM services and patient counseling while playing a key role in launching and sustaining a clinical pharmacy initiative. Candidates must reside in California and already be enrolled in Medicaid as an Ordering, Referring, and Prescribing (ORP) provider.
Location: Remote within California (candidate must be a CA resident)
Schedule:
- Approximately 10-20 hours per week
- Day shift hours required for patient outreach and counseling
- Guaranteed minimum of 8 paid hours per week regardless of workload
Responsibilities:
- Conduct Medication Therapy Management consultations
- Provide patient counseling and medication education
- Review medication profiles to identify opportunities for optimization
- Accurately document clinical interventions and patient encounters
- Collaborate with program stakeholders to improve patient outcomes
- Ensure compliance with state and federal pharmacy regulations
Qualifications:
- Active and unrestricted California Pharmacist license
- Residency within California is required
- Experience with Medication Therapy Management
- Current enrollment in California Medicaid (Medi-Cal) as an ORP provider is mandatory
- Excellent communication and patient counseling skills
- Ability to work independently in a remote environment and meet scheduling needs
