This pharmacy opportunity focuses on sterile compounding within the home infusion setting, providing specialized medication therapy and patient care. Located in the Cibolo, TX area, this 13-week contract role offers a consistent 12 PM to 8 PM schedule, ideal for pharmacists with a strong foundation in sterile IV compounding and home infusion therapy.

Key Responsibilities:

  • Assess patients for eligibility in home infusion therapy and develop tailored medication therapy plans based on clinical data and pharmacokinetics.
  • Prepare, verify, and dispense sterile medications following USP guidelines, state and federal regulations, and internal pharmacy policies.
  • Educate patients, caregivers, and healthcare professionals on medication use, infusion administration, and therapy management.
  • Collaborate with healthcare providers to ensure seamless continuity of care while monitoring patient outcomes.
  • Manage infusion pumps and related equipment, maintaining strict quality assurance and safety measures.
  • Provide accurate drug information, maintain detailed documentation, and contribute to regulatory compliance and medication safety protocols.

Qualifications and Desired Experience:

  • Doctor of Pharmacy (PharmD) degree or graduation from an accredited School of Pharmacy.
  • Active, unrestricted Texas Pharmacist license.
  • One or more of the following: minimum 1 year sterile IV compounding experience; non-sterile compounding, immunizations, and Medication Therapy Management (MTM) exposure; completion of ASHP-accredited residency; completion of Advanced Pharmacy Practice Experience (APPE); or equivalent relevant experience.
  • Strong clinical knowledge of home infusion pharmacy practices.
  • Proficient in interpreting laboratory values and applying pharmacokinetic principles.
  • Excellent communication, patient counseling, and customer service skills.
