Pharmacist in Collierville, TN

A direct hire opportunity is available for an Infusion/Clinical Pharmacist at a facility near Bartlett/Memphis, TN. This role requires a pharmacist with specialized skills in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications within a home infusion setting. The position offers a Monday through Friday schedule, from 8 a.m. to 5 p.m., with the expectation to participate in an on-call rotation averaging fewer than five after-hours calls per week.

Location: Bartlett/Memphis, TN
Schedule:

• Monday - Friday, 8:00 a.m. - 5:00 p.m.
• On-call rotation (fewer than 5 after-hours calls per week)

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions in compliance with state and federal regulations.
• Monitor lab values and pharmacokinetic dosing for medications such as vancomycin, and manage Total Parenteral Nutrition (TPN).
• Provide education and counseling to patients and caregivers on specialty medications and administration techniques.
• Collaborate with prescribing physicians, home health nurses, and internal teams to optimize therapy and prevent hospital readmissions.
• Ensure adherence to regulatory requirements including HIPAA, FDA, DEA, and OSHA standards.
• Manage pharmacy inventory, including refrigerated items, and coordinate the packaging and timely delivery of medications to patients’ homes.

Desired Qualifications:

• Minimum of 3 years’ experience in infusion specialty pharmacy.
• Experience with IVIg and hemophilia treatments is highly preferred.
• Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and active Tennessee pharmacist license in good standing.
• Familiarity with the CareTend system is a plus.
