Clinical Infusion Pharmacist

Soliant Health is seeking an experienced Clinical/Home Infusion Pharmacist for a direct-hire/permanent opportunity supporting home infusion operations in Columbus, Ohio. This pharmacist will play a key clinical role in managing complex IV and specialty therapies while collaborating with physicians, nurses, patients, caregivers, and internal clinical teams to ensure safe and effective patient care.

• Employment Type: Full-time | Direct Hire/Permanent
• Schedule: Monday–Friday, 8:30 AM–5:00 PM
• On-Call: Participation in rotation required

Responsibilities

• Prepare and dispense sterile compounded IV and specialty medications.
• Monitor drug therapies, lab values, pharmacokinetic dosing, and TPN, making clinical recommendations as needed.
• Provide medication counseling and education to patients and caregivers on home infusion therapies and equipment.
• Collaborate with physicians, nurses, and internal clinical and operational teams to coordinate patient care.
• Maintain compliance with pharmacy, healthcare, and regulatory standards.
• Assist with pharmacy inventory and preparation of medications and supplies for safe home delivery. 

Qualifications

• Ohio Pharmacist license in good standing.
• Minimum 2 years of infusion or specialty pharmacy experience.
• Experience with sterile compounding and complex IV therapies.
• Experience with clinical monitoring, pharmacokinetic dosing, and TPN.
• IVIg experience (highly preferred).
• Knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations (preferred).
• Experience with CareTend software (preferred).
