Pharmacist - Converse, TX

A Sterile Compounding Pharmacist opportunity is available for a 13-week assignment in Converse, TX, with a schedule from 12 PM to 8 PM. This position involves specialized responsibilities in home infusion therapy within a contract role.

Responsibilities include evaluating patients for home infusion therapy eligibility, developing and adjusting individualized medication plans based on clinical and pharmacokinetic assessments, and preparing sterile medications in compliance with USP standards and applicable regulations. The pharmacist will also educate patients, caregivers, and healthcare providers on medication use and infusion administration. Maintaining continuity of care through collaboration, managing infusion equipment, and ensuring safety and quality assurance are critical. Additional duties involve providing drug information, maintaining accurate records, supporting regulatory compliance, and promoting medication safety initiatives.

Qualifications:

• Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
• Active, unrestricted Pharmacist license in Texas.
• One of the following:
  • Minimum 1 year of sterile IV compounding experience.
  • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
  • Completion of an ASHP-accredited Pharmacy Residency.
  • Completion of an Advanced Pharmacy Practice Experience (APPE).
  • Equivalent pharmacy experience.
