A professional opportunity is available for a Sterile Compounding Pharmacist to support home infusion therapy services. This contract position offers a 13-week assignment with a schedule from 12 PM to 8 PM in Converse, TX, providing a focused and impactful role within pharmaceutical care.
Key Responsibilities:
- Evaluate patients for eligibility and suitability for home infusion therapy.
- Develop, monitor, and adjust personalized medication therapy plans using clinical assessments, lab data, and pharmacokinetic principles.
- Prepare, verify, and dispense sterile medications complying with USP standards and relevant regulations.
- Educate patients, caregivers, and healthcare providers on infusion administration and optimal medication use.
- Collaborate with healthcare teams to ensure continuity of care and monitor patient outcomes.
- Manage infusion pumps and medication administration equipment adhering to quality assurance and safety standards.
- Provide accurate drug information, maintain thorough documentation, and support medication safety and regulatory compliance initiatives.
Qualifications and Experience:
- Doctor of Pharmacy (PharmD) or graduation from an accredited School of Pharmacy.
- Active, unrestricted Pharmacist license in Texas.
- One or more of the following:
- Minimum of 1 year sterile IV compounding experience,
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
- Completion of an ASHP-accredited Pharmacy Residency,
- Completion of an Advanced Pharmacy Practice Experience (APPE),
- Or equivalent pharmacy experience.
Required Skills:
- Strong clinical expertise in home infusion pharmacy practices.
- Proficient in interpreting laboratory values and applying pharmacokinetic principles.
- Excellent communication, patient counseling, and customer service abilities.
