Pharmacist | Cordova, Tennessee

A direct hire opportunity is available for an experienced Infusion/Clinical Pharmacist to join a dynamic team providing specialized home infusion pharmacy services in the Cordova, TN area, serving the Bartlett/Memphis region. This role is integral in preparing, dispensing, and clinically monitoring complex intravenous (IV) and subcutaneous medications, supporting patients through education, medication management, and care coordination.

Location: Cordova, TN (serving Bartlett/Memphis, TN area)
Schedule:

• Monday to Friday, 8:00 AM – 5:00 PM
• Participate in an on-call rotation, typically handling fewer than 5 after-hours calls per week
  Start: ASAP

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions, complying with all state and federal regulations
• Monitor clinical parameters, perform pharmacokinetic dosing calculations (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) as needed
• Provide thorough education to patients and caregivers on infusion pump use, sterile techniques, and catheter care at home
• Collaborate closely with physicians, home health nurses, and internal teams including intake, referrals, reimbursement, clinical services, and pharmacy operations to ensure optimal therapy management and reduce hospital readmissions
• Ensure compliance with all relevant regulatory standards, including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA
• Manage pharmacy inventory, including refrigerated items, and prepare medication shipments for timely and safe delivery

Qualifications & Experience:

• Minimum 3 years of infusion specialty pharmacy experience
• Experience with IVIg and hemophilia therapies highly preferred
• Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations
• Active, current Tennessee pharmacist license in good standing
• Familiarity with the CareTend system preferred
