A direct hire opportunity is available for an experienced Infusion/Clinical Pharmacist at a dynamic home infusion pharmacy in the Cordova, TN area, serving the Bartlett/Memphis communities. This role involves preparing and monitoring complex IV and subcutaneous medications while playing an integral part in a multidisciplinary healthcare team.
Schedule:
- Monday to Friday, 8:00 AM - 5:00 PM
- Participate in on-call rotation (less than 5 after-hours calls weekly)
Key Responsibilities:
- Prepare sterile compounded specialty pharmaceuticals and intravenous solutions in compliance with state and federal regulations.
- Monitor clinical parameters such as lab values and pharmacokinetic dosing, including vancomycin, and adjust Total Parenteral Nutrition (TPN) regimens.
- Provide detailed counseling and education to patients and caregivers on infusion pumps, sterile technique, and catheter maintenance at home.
- Collaborate closely with prescribing physicians, home health nurses, and internal teams to coordinate care and reduce hospital readmissions.
- Ensure adherence to regulatory requirements including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA standards.
- Manage inventory, including refrigerated products, and oversee packaging and shipping to ensure timely and safe delivery of medications.
Qualifications and Desired Experience:
- Minimum of 3 years’ experience in infusion specialty pharmacy.
- Experience with IVIg and hemophilia treatments highly preferred.
- Familiarity with specialty therapy payer requirements, reimbursement procedures, and contract obligations.
- Current and valid Tennessee pharmacist license in good standing.
- Proficient in CareTend system or similar pharmacy management software.
