Two direct hire opportunities are available for experienced inpatient pharmacists specializing in oncology and hematology. These roles focus on providing expert pharmaceutical care to patients within an acute care setting and involve collaboration with multidisciplinary teams to optimize medication regimens.
Location: Des Plaines, IL
Schedule:
- Typical inpatient schedule
- Days and evenings
- Rotating weekends
Key Responsibilities:
- Attend interdisciplinary rounds for malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) patients to support medication regimen design, implementation, and monitoring
- Review drug therapies for appropriate use, dosing, routes, and potential interactions
- Reconcile home medications for admitted patients
- Collaborate with prescribers and healthcare teams to clarify medication orders and document changes
- Provide patient and family counseling on medication adherence and adverse effects, especially prior to discharge of SCT and other appropriate patients
- Conduct pharmacokinetic consultations, including documentation for vancomycin and aminoglycoside dosing
- Monitor, document, and report adverse drug reactions and medication errors
- Verify chemotherapy orders for oncology patients
- Offer support to nursing staff regarding medication inquiries
- Participate in medication order verification and related pharmacy communications
- Assist with chemotherapy desensitizations in critical care settings
Desired Experience / Qualifications:
- Bachelor of Science in Pharmacy required
- Active Illinois Pharmacist License or eligibility to obtain one
- Post Graduate ASHP Year Two Pharmacy Residency in Hematology/Oncology highly preferred
- Board Certification in Oncology Pharmacy (BCOP) prior to or within 18 months of hire strongly preferred
- BLS Certification
- Solid foundation in intravenous admixture, drug therapy, chemotherapy, and cellular therapies
- Strong problem-solving skills and ability to work independently in a fast-paced environment
- Excellent teamwork and communication abilities
Candidates with a strong oncology background, even if not residency-trained, will be considered.
