Two direct hire opportunities are available for inpatient Oncology/Hematology pharmacists with a healthcare organization in Des Plaines, IL. These roles focus on providing expert pharmacy services within an inpatient acute care oncology setting, supporting teams treating malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) patients. The positions require a dedication to collaborative patient care and medication safety, with an emphasis on specialized oncology pharmacy practice.

Responsibilities include:

  • Participating in interdisciplinary rounds to assist in medication regimen design, implementation, and monitoring
  • Reviewing drug therapies for appropriate use, dosing, and potential interactions
  • Reconciling home medications upon patient admission
  • Clarifying medication orders with healthcare providers and documenting changes
  • Providing patient and family counseling on medication regimens and adherence, particularly for SCT patients
  • Conducting pharmacokinetic consultations and documenting vancomycin and aminoglycoside therapy
  • Monitoring, documenting, and reporting adverse drug reactions and medication errors
  • Reviewing chemotherapy orders for oncology and SCT inpatients
  • Supporting nursing staff with medication-related inquiries and order verifications
  • Assisting in chemotherapy desensitizations and converting medications from IV to oral when appropriate
  • Staffing evening shifts as needed

Desired qualifications include:

  • Bachelor of Science in Pharmacy
  • Illinois Pharmacist License or eligibility to obtain
  • Postgraduate ASHP Year Two Pharmacy Residency in Hematology/Oncology (highly preferred)
  • Board Certification in Oncology Pharmacy (BCOP) prior to or within 18 months of hire
  • Basic Life Support (BLS) certification
  • Knowledge of intravenous admixture services, chemotherapy protocols, and cellular therapies
  • Strong problem-solving skills and ability to work independently in a fast-paced environment
  • Experience working effectively within interdisciplinary teams

A typical inpatient schedule includes days and evenings with rotating weekends. Candidates with solid oncology foundations, including those with some oncology experience, are encouraged to apply. The employer is open to supporting candidates who are willing to pursue Illinois licensure.
