Per Diem Pharmacist | Durham, North Carolina

An opportunity is available for a Pharmacist with expertise in home infusion pharmacy to provide per diem coverage from July 29 through August 5. This role offers weekday hours from 9 a.m. to 5 p.m., Monday through Friday, supporting pharmaceutical services in a home infusion environment.

Key Responsibilities:

• Perform thorough verification of pharmaceutical products entering the clean room for sterile compounding, ensuring accuracy with unique lot numbers on batch records.
• Conduct final checks on compounded sterile preparations (CSP), including labels, fill amounts, and drugs used before release.
• Inspect final shipments sent to patients, confirming all medications and medical supplies listed on delivery tickets are included.
• Communicate effectively with medical doctors, field nurses, and patients regarding drug regimens, inventory management, potential drug reactions, and abnormal laboratory findings.
• Proactively contact patients to coordinate medication deliveries, assess inventory levels, monitor medication adherence, and report any concerns to management.
• Manage inbound calls from physician offices, nursing agencies, and patients, addressing inquiries and providing support as needed.

Qualifications:

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy or Master of Science in Pharmacy preferred from an accredited institution.
• Active pharmacist license in North Carolina.
• Minimum two years of recent experience in an acute care pharmacy setting or one year in home infusion therapy.
• Strong knowledge and hands-on experience in intravenous admixture services and intravenous therapy.
• Proficiency with CPR+ software is mandatory.
