Pharmacist | El Cajon, California

A California-licensed Pharmacist is needed to support a remote Medication Therapy Management (MTM) pilot program. This part-time contract role offers flexible scheduling (10-20 hours per week) and the opportunity to work remotely from anywhere within California. The pharmacist will contribute to patient counseling, MTM consultations, and the launch of a clinical pharmacy initiative focused on improving patient outcomes.

Location:

• Remote (California residents only)

Schedule/Hours:

• Flexible scheduling, approximately 10-20 hours per week
• Day shift hours required for patient outreach and counseling
• Guaranteed minimum of 8 paid hours per week regardless of workload

Responsibilities:

• Conduct Medication Therapy Management (MTM) consultations
• Provide patient counseling and medication education
• Review medication profiles to identify optimization opportunities
• Document clinical interventions and patient encounters thoroughly and accurately
• Collaborate with program stakeholders to support patient outcomes
• Ensure compliance with all applicable state and federal regulations

Qualifications:

• Active and unrestricted California Pharmacist license
• Must reside in California
• Previous MTM experience required
• Enrollment as an Ordering, Referring, and Prescribing (ORP) provider in California Medicaid (Medi-Cal) is mandatory
• Strong communication and patient counseling skills
• Ability to work independently in a remote setting with flexible hours

This position is a 3-6 month contract with potential extension depending on the pilot’s progression. Candidates will have the chance to discuss role specifics, technology, and workflows during the interview process.
