A California-licensed pharmacist residing in California is sought to provide remote Medication Therapy Management (MTM) services as part of an innovative clinical pilot program. This part-time contract role offers flexible scheduling with approximately 10-20 hours per week, primarily during day shifts, supporting patient outreach and counseling efforts from the comfort of your home.
Key Responsibilities:
- Conduct MTM consultations to improve patient medication outcomes
- Provide clear patient counseling and medication education
- Review medication profiles to identify and recommend optimization opportunities
- Accurately document clinical interventions and patient interactions
- Collaborate with stakeholders to enhance program success and patient care
- Ensure compliance with all relevant state and federal pharmacy regulations
Qualifications and Experience:
- Active, unrestricted California Pharmacist license
- Current residency within California
- Previous MTM experience is essential
- Enrollment as an Ordering, Referring, and Prescribing (ORP) provider with California Medicaid (Medi-Cal) is mandatory
- Excellent communication and patient counseling abilities
- Comfortable working independently and managing flexible remote hours
Schedule:
- Approximately 10-20 hours weekly, flexible day shift hours to align with patient needs
- Guaranteed minimum of 8 paid hours per week regardless of workload
