A California-licensed Pharmacist is needed to support a remote Medication Therapy Management (MTM) pilot program. This part-time contract position offers flexible hours (10-20 hours per week) with a guaranteed minimum of 8 paid hours weekly. Candidates must currently reside in California and be actively enrolled in Medicaid as an ORP (Ordering, Referring, and Prescribing) provider. This opportunity allows you to work remotely from anywhere within California while contributing to a clinical pharmacy initiative aimed at improving patient outcomes.

Schedule:
- Flexible scheduling, approximately 10-20 hours per week
- Day shift hours required for patient outreach and counseling
- Guaranteed minimum of 8 paid hours per week, regardless of workload
Responsibilities:
- Conduct Medication Therapy Management (MTM) consultations
- Provide patient counseling and medication education
- Review medication profiles to identify optimization opportunities
- Accurately document clinical interventions and patient encounters
- Collaborate with program stakeholders to support patient outcomes
- Ensure compliance with all applicable state and federal regulations
Qualifications:
- Active, unrestricted California Pharmacist license
- Residency in California is mandatory
- Previous MTM experience required
- Current enrollment in California Medicaid (Medi-Cal) as an ORP provider
- Strong communication and patient counseling skills
- Ability to work independently in a remote setting with flexible hours
