Pharmacist | Elgin, Oklahoma

An exciting opportunity is available for a Pharmacist to join a healthcare team dedicated to diabetes management within the Lawton Service Unit Diabetes Pharmacy Clinics in the Oklahoma City area. This contract position offers hands-on experience in a collaborative pharmacy practice environment focused on patient-centered diabetes care.

Location: Lawton, OK, with some floating required to Carnegie and Anadarko
Schedule: 8-hour day shifts
Duration: 1-year contract with four additional 1-year extension options

Key Responsibilities:

• Process daily prescription orders from providers, accurately fill prescriptions, and dispense new medications and refills
• Provide thorough patient counseling on medications to promote adherence and understanding
• Oversee pharmacy support personnel, ensuring smooth workflow and compliance with protocols
• Practice intensive diabetes management based on a comprehensive pharmacy collaborative agreement within the Lawton Service Unit
• Utilize evidence-based guidelines, including the American Diabetes Association Standards of Care, to inform clinical decisions
• Demonstrate proficiency in the placement and data interpretation of continuous glucose monitors (CGMs), facilitating appropriate medication adjustments

Qualifications and Desired Experience:

• Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD) degree
• Any state pharmacy license is acceptable, reflecting the inclusive licensing policy
• Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) OR completion of a Post Graduate Year One (PGY1) residency program
