Per Diem Pharmacist in Elk River, MN

A specialty pharmacy is seeking a knowledgeable Pharmacist to provide expert pharmaceutical care in a combined retail and long-term care setting. Schedule:

• 8/26: 8:30 AM – 6:00 PM
• 8/27: 8:30 AM – 6:00 PM
• 8/28: 8:30 AM – 6:00 PM
• 9/1: 8:30 AM – 6:00 PM

Responsibilities:

• Dispense medications accurately in a retail pharmacy setting
• Manage and oversee long-term care pharmacy services
• Provide immunizations and patient counseling
• Ensure compliance with pharmacy laws and regulations
• Collaborate with healthcare professionals to optimize patient outcomes
• Conduct medication therapy management and review

Qualifications:

• PharmD degree preferred
• Immunization certification required
• CPR certification required
• Experience and comfort with long-term care pharmacy duties
• Medication Therapy Management (MTM) certification is a plus
• Excellent communication and attention to detail
