Key Responsibilities:
- Participate in medical rounds to assess and monitor patient medication regimens.
- Provide pharmacokinetic dosing and counsel patients regarding their therapy.
- Monitor and manage patient drug therapy for safety and efficacy.
- Support unit dose and intravenous (IV) services to ensure proper medication distribution.
- Collaborate with healthcare professionals to improve treatment outcomes.
Qualifications:
- Minimum of 2 years’ experience as a clinical hospital pharmacist.
- Experience in rounding with medical teams and patient counseling is highly desirable.
- Skilled in pharmacokinetic dosing and managing complex drug therapies.
- Current Registered Pharmacist license in the state of Hawaii.
- Doctor of Pharmacy (PharmD) degree preferred.
- Completion of an ASHP-accredited post-graduate clinical residency is a plus.
Schedule and Location:
- Location: Honolulu, HI
- Duration: 16 weeks, with flexible start date ASAP.
- Rotating shifts including day and evening hours: • Day Shift Rotation: Five 8-hour shifts per week (0630-1500 or 0830-1700) • Evening Shift Rotation: Four 10-hour shifts per week (1100-2130 or 1230-2300)
- 40 hours guaranteed weekly.
