An opportunity is available for a skilled pharmacist to support clinical pharmacy services in Honolulu, HI. This travel position offers a dynamic work schedule with rotations across day and evening shifts, providing the chance to engage in diverse clinical activities within a hospital setting.
Responsibilities:
- Collaborate with healthcare teams during medical rounds to optimize patient drug therapy.
- Monitor patients’ pharmacokinetic dosing, adjusting medications as needed.
- Provide patient counseling and education on medication use and safety.
- Manage unit dose and IV medication services to ensure accurate dispensing and preparation.
- Maintain compliance with all regulatory and institutional pharmacy standards.
Qualifications:
- Minimum of 2 years experience as a Clinical Hospital Pharmacist.
- Experience in rounding with medical teams, patient drug therapy monitoring, pharmacokinetic dosing, and patient education.
- Familiarity with unit dose and intravenous (IV) pharmacy services preferred.
- Current Hawaii State Registered Pharmacist license required.
- Doctor of Pharmacy (PharmD) degree preferred.
- Completion of a post-graduate clinical residency accredited by the American Society of Health-System Pharmacists is a plus.
Schedule:
- 40 hours per week guaranteed.
- Rotating shifts between day and evening schedules.
- Day Shift: Five 8-hour shifts per week (06:30–15:00 or 08:30–17:00).
- Evening Shift: Four 10-hour shifts per week (11:00–21:30 or 12:30–23:00).
