Pharmacist - Faribault, Minnesota

A specialty pharmacy is seeking a skilled Pharmacist to provide expert care in a long-term care setting. This contract opportunity offers a chance to utilize your clinical expertise in a dynamic environment focused on specialized medication management and patient care.

Location: Faribault, MN
Duration: 13+ weeks
Hours: 24-40 hours per week
Schedule:

• Monday to Friday, 8:30 AM – 5:30 PM
• Some weekends required

Responsibilities:

• Perform long-term care pharmacist duties, ensuring accurate medication dispensing and patient safety
• Provide medication therapy management and counseling to patients and healthcare providers
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Maintain compliance with pharmacy regulations and standards
• Support immunization initiatives as needed

Qualifications/Desired Experience:

• PharmD degree or equivalent professional pharmacy degree
• Immunization Certification
• CPR Certification
• Medication Therapy Management (MTM) Certification preferred
• Strong knowledge of long-term care pharmacy practices
• Excellent communication and organizational skills
