Responsibilities:

• Dispense medications accurately and efficiently, ensuring compliance with all regulations.
• Collaborate closely with healthcare teams to develop and review patient medication plans.
• Monitor patient responses to medications and make necessary adjustments.
• Provide medication counseling and support to patients and healthcare staff.
• Ensure proper inventory management and medication storage procedures.
• Participate in quality assurance and safety initiatives specific to LTAC care.

Desired Experience:

• Current licensure as a Pharmacist.
• Proven experience in pharmacy operations within an LTAC or similar healthcare setting.
• Strong knowledge of medication therapy management, drug formulations, and potential interactions.
• Excellent communication skills for patient and team interactions.
• Ability to work independently while maintaining strong collaboration with medical staff.
