This opportunity is for a Pharmacist to support a small Long-Term Acute Care (LTAC) hospital in Forest, VA. The facility has a daily census of approximately 10 patients, providing focused and meaningful patient care. This position is a contract role, starting ASAP, with an expected duration of 13 weeks or more.
Key Responsibilities:
- Dispense and manage medications accurately for LTAC patients.
- Collaborate closely with the healthcare team to optimize medication therapy.
- Monitor patient progress and adjust treatments based on clinical response and safety.
- Ensure compliance with hospital policies, regulatory standards, and pharmacy best practices.
- Maintain accurate and thorough documentation of all pharmacy activities.
Qualifications:
- Active Virginia Pharmacist license required.
- Experience in a hospital setting preferred, ideally with LTAC exposure.
- Strong communication skills to effectively interact with multidisciplinary teams.
- Detail-oriented with excellent organizational and problem-solving abilities.
- Ability to work independently while adhering to clinical protocols and guidelines.
Schedule:
- Monday through Friday
- Day shift hours
