A new pharmacist opportunity is available at a Diabetes Pharmacy Clinic serving the Oklahoma City area, with a primary focus on Lawton, OK. This contract position involves supporting patient care through diabetes medication management and collaborating within a pharmacy-based practice agreement. The role requires some travel to nearby sites including Carnegie and Anadarko.

Location: Lawton, Oklahoma (with float to Carnegie and Anadarko)
Type: Contract (1 year with four 1-year extension options)
Schedule: 8-hour day shifts
Start Date: Immediate

Key Responsibilities:

  • Process daily prescription orders from providers, including prescription filling and dispensing of medications and refills.
  • Provide extensive patient counseling on medication use, particularly diabetes therapies.
  • Supervise pharmacy support staff to ensure efficient operation of the clinic.
  • Practice intensive diabetes management in accordance with a collaborative pharmacy-based agreement.
  • Use evidence-based clinical guidelines such as the American Diabetes Association Standards of Care to guide decision-making.
  • Demonstrate proficiency with continuous glucose monitors (CGMs), including placement, data extraction, interpretation, and medication dose adjustments.

Required Qualifications:

  • Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD).
  • Pharmacist license from any state is acceptable.
  • Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS), or completion of a Post Graduate Year One (PGY1) residency.
