Join a dedicated healthcare team focused on advancing diabetes care through pharmacy-based services at a respected service unit in Lawton, OK. This contract role offers an opportunity to practice specialized diabetes management while supporting patient health in a community setting.

Located primarily in Lawton, with some flexibility to float to Carnegie and Anadarko, this position involves 8-hour day shifts with a start date as soon as possible. The initial commitment is one year with up to four one-year extension options available.

Key Responsibilities:

  • Accurately process and fill daily prescription orders, including new medications and refills, generated by healthcare providers.
  • Deliver detailed patient counseling on medication use and management.
  • Supervise and support pharmacy personnel to ensure efficient operations.
  • Implement intensive diabetes management protocols in accordance with a collaborative practice agreement.
  • Utilize evidence-based guidelines, such as the American Diabetes Association Standards of Care, to guide clinical decisions.
  • Expertly place continuous glucose monitors (CGMs) and interpret data to optimize patient medication regimens.

Qualifications and Experience:

  • Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD) required.
  • Licensure from any state is accepted, reflecting inclusive credentialing.
  • Board Certification (BCACP or BCPS) or completion of a Post Graduate Year One residency preferred.
