A rewarding opportunity is available for a skilled pharmacist to join a Long-Term Acute Care (LTAC) setting on a 13-week contract starting ASAP. This role requires a healthcare professional who excels in clinical pharmacy practice, particularly within hospital or LTAC environments, and possesses strong communication and medication management expertise.
Key Responsibilities:
- Perform medication reconciliation to develop accurate discharge medication lists
- Collect prior-to-admission home medication information from patients, families, or retail pharmacies
- Provide thorough discharge counseling to patients and families, ensuring clarity and adherence
- Support Antimicrobial Stewardship by interpreting culture results and making evidence-based recommendations for infections including UTIs, pneumonia, and skin and soft tissue infections (SSTIs)
- Manage and monitor pharmacokinetic dosing and anticoagulation therapy
- Utilize EPIC system effectively for patient care documentation and workflow
Desired Qualifications:
- Previous hospital or LTAC experience is required
- Certification in Basic Life Support (BLS)
- Strong clinical knowledge of antimicrobial therapies and pharmacokinetics
- Excellent communication skills to engage patients and interdisciplinary teams effectively
- Proficiency with EPIC electronic health records preferred
