A pharmacy role is available at a 400-bed hospital in the San Fernando Valley Region of Los Angeles, focusing on Transitions of Care and Ambulatory Care. This position offers the opportunity to work closely with patients as they prepare for discharge, ensuring medication education and counseling are provided to enhance adherence to prescribed regimens and support overall health outcomes.

Responsibilities:

  • Collaborate with patients prior to discharge to deliver medication education and counseling
  • Facilitate medication refills before patient discharge as part of the 'meds to beds' program
  • Support transitions of care to improve patient compliance with medication therapies
  • Work within a multidisciplinary team in a large health system serving diverse ambulatory and inpatient populations

Qualifications and Desired Experience:

  • Licensed pharmacist with a strong interest in ambulatory and transition of care pharmacy
  • PGY1 residency graduates preferred but candidates without PGY1 training will be considered
  • Ability to effectively communicate and educate patients from various backgrounds
  • Experience or familiarity with hospital and outpatient pharmacy operations advantageous
  • Commitment to patient-centered care and improving health outcomes through medication management
