A travel pharmacist position is available in Northern Wisconsin to support a 50-bed hospital. This opportunity begins ASAP with a contract duration of 13 weeks or more. The schedule follows a 7 days on, 7 days off rotation with 12-hour day shifts, providing an excellent work-life balance for pharmacists seeking a focused, immersive experience in a hospital setting.

Responsibilities:

• Review medication orders to ensure appropriate drug therapy, assessing dose, dosage form, regimen, therapeutic duplication, and potential drug interactions.
• Provide pharmacokinetic and anticoagulation dosing support.
• Participate in delivering pharmaceutical care, which includes medication reconciliation and educating patients on discharge medications.
• Oversee pharmaceutical compounding activities, ensuring adherence to USP regulatory and compliance guidelines, with a focus on USP 797 and USP 800 standards.
• Utilize EPIC electronic health record system for documentation and workflow integration.

Desired Experience:

• Licensed pharmacist eligible to practice in the state.
• Experience in hospital pharmacy practice, preferably in similar-sized facilities.
• Skilled in medication order review and clinical pharmaceutical care.
• Knowledge and experience with pharmacokinetic dosing and anticoagulation management.
• Familiarity with pharmaceutical compounding regulations, specifically USP 797 and USP 800.
• Proficiency with EPIC or similar electronic health record systems.
