Pharmacist - Glidden, Wisconsin

A 13-week contract opportunity is available for a Pharmacist at a 50-bed hospital in Northern Wisconsin. This role offers the chance to contribute to a close-knit clinical team, providing essential pharmaceutical care through a variety of patient-focused services.

Key responsibilities include:

• Reviewing medication orders for drug therapy accuracy, including appropriateness of use, dosing, dosage form, regimen, duplication, and drug interactions
• Providing pharmacokinetic and anticoagulation dosing expertise
• Participating in clinical services such as medication reconciliation and discharge medication education
• Overseeing pharmaceutical compounding while ensuring compliance with USP 797 and USP 800 regulatory guidelines
• Utilizing EPIC for clinical documentation and order processing

Schedule:

• 12-hour day shifts
• 7 days on, 7 days off rotation

Desired Qualifications:

• Licensed Pharmacist with relevant experience in hospital pharmacy practice
• Strong knowledge of pharmacokinetics and anticoagulation management
• Familiarity with USP 797 and USP 800 standards for pharmaceutical compounding
• Experience working with EPIC or similar electronic health record systems
• Ability to work independently and as part of a healthcare team
