TWO Direct Hire Opportunities available for inpatient oncology/hematology pharmacists at a leading medical center located in Illinois. These roles require immediate start and offer the chance to work in a highly specialized and collaborative inpatient setting, supporting malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) teams.

Schedule: Typical inpatient rotation with days/evenings and rotating weekends

Key Responsibilities:
- Participate in interdisciplinary rounds to design, implement, and monitor complex medication regimens for oncology and SCT patients.
- Review and verify chemotherapy orders ensuring the appropriateness, safety, and cost-effectiveness of drug therapies.
- Reconcile home medications upon patient admission and communicate medication clarifications with prescribers and healthcare teams.
- Provide patient and family counseling on medication regimens, adherence, and potential adverse effects.
- Perform pharmacokinetic consultations and document appropriately, particularly for vancomycin and aminoglycosides.
- Monitor, document, and report adverse drug reactions and medication errors.
- Assist nursing staff with medication order questions and facilitate chemotherapy desensitizations in critical care as needed.
- Engage in evening staffing based on departmental needs.
Desired Qualifications:
- Bachelor of Science in Pharmacy with Illinois Pharmacist License or eligibility to obtain licensure.
- Postgraduate ASHP-accredited PGY2 Hematology/Oncology Residency strongly preferred; however, candidates with solid oncology experience and foundation will be considered.
- Board Certified Oncology Pharmacist (BCOP) certification expected within 18 months of hire.
- BLS Certification.
- Proficiency in intravenous admixture, intravenous drug therapy, chemotherapy, and cellular therapies.
- Excellent independent problem-solving and communication skills to provide safe and effective drug therapy in a fast-paced environment.
