A new opportunity is available for a Pharmacist specializing in diabetes management at a healthcare service unit serving Lawton, OK, with some travel required to nearby locations. This contract role offers an 8-hour day-shift schedule and is initially for one year, with the possibility of four additional one-year extensions.

Location:

  • Lawton, OK, with occasional float to Carnegie and Anadarko

Schedule:

  • 8-hour day shifts

Responsibilities:

  • Process daily prescription orders accurately from healthcare providers, including filling and dispensing both new prescriptions and refills.
  • Provide extensive patient counseling focused on medication adherence and safety.
  • Supervise and support pharmacy support personnel to ensure smooth operations.
  • Deliver specialized diabetes care according to a collaborative practice agreement, utilizing evidence-based guidelines such as the American Diabetes Association Standards of Care.
  • Demonstrate expertise in continuous glucose monitor (CGM) placement, data extraction, and interpretation to optimize medication adjustments.

Qualifications:

  • Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD) degree required.
  • Possession of any state pharmacy license is acceptable, reflecting the inclusive licensing requirements.
  • Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy Specialties, OR successful completion of a Post Graduate Year One (PGY1) residency program.
