Join a dynamic healthcare setting as a Pharmacist focused on diabetes management within a well-established service unit. This contract role, based in Lawton, OK with occasional travel required to nearby locations such as Carnegie and Anadarko, offers the opportunity to apply clinical expertise in a community-oriented environment. Ideal candidates will have a passion for patient care and advanced knowledge in diabetes therapy.

Key Responsibilities:

  • Process daily prescription orders and efficiently fill and dispense both new prescriptions and refills.
  • Provide extensive patient counseling to optimize medication adherence and therapeutic outcomes.
  • Oversee pharmacy support staff to ensure high standards of service and accuracy.
  • Deliver intensive diabetes management under a collaborative pharmacy practice agreement.
  • Employ decision-making skills aligned with the American Diabetes Association Standards of Care.
  • Utilize continuous glucose monitors (CGMs) proficiently, including placement, data extraction, and interpretation to guide medication adjustments.

Qualifications and Experience:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree required.
  • Any state pharmacy license is acceptable.
  • Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) or completion of a Post Graduate Year One residency program preferred.

Location:

  • Lawton, OK, with some float to Carnegie and Anadarko.

Schedule:

  • Full-time, 8-hour day shifts.
