A growing healthcare provider is seeking a dedicated bilingual Clinical Pharmacist (Spanish & English) to join their pharmacy team in Van Nuys, CA. This contract-to-perm opportunity offers the chance to contribute your clinical expertise within a patient-focused environment, emphasizing quality care and long-term professional growth. The pharmacy staff includes pharmacists, pharmacy technicians, and clinical support personnel working collaboratively to optimize chronic disease management, particularly diabetes care.

Location: Van Nuys, CA
Schedule:

  • Monday to Friday, 9:00 AM – 6:00 PM
  • Rotating every third Saturday, 9:00 AM – 1:00 PM
    Contract Duration: 13 weeks (Temp-to-Perm)

Key Responsibilities:

  • Perform retail pharmacy duties with significant clinical involvement, focusing heavily on medication review and patient counseling
  • Verify and dispense approximately 250 prescriptions daily, ensuring accuracy, safety, and regulatory compliance
  • Conduct comprehensive clinical medication reviews to assess appropriateness, interactions, dosing, and therapeutic outcomes
  • Deliver extensive patient counseling emphasizing diabetes management, including insulin use, injectables, continuous glucose monitors (CGMs), oral agents, and lifestyle guidance
  • Monitor chronic disease states through clinical assessments, collaborating closely with prescribers to clarify orders and optimize therapies
  • Identify and resolve medication-related issues and adherence challenges
  • Oversee pharmacy workflow to maintain efficiency and compliance
  • Foster a patient-centered atmosphere committed to long-term health outcomes and quality care

Desired Qualifications:

  • B.S. or PharmD from an accredited pharmacy school
  • Active and clear California Pharmacist License
  • Minimum 2 years of clinical pharmacy experience preferred
  • Experience in diabetic clinical assessment highly valued
  • Bilingual proficiency in Spanish and English strongly preferred
  • Reliable, professional, patient-focused with strong clinical judgment and counseling skills
