A California-licensed Pharmacist is needed for a remote Medication Therapy Management (MTM) pilot program designed to enhance clinical pharmacy services. This part-time contract position offers flexible scheduling with approximately 10-20 hours per week, allowing you to work remotely anywhere within California. The program emphasizes patient counseling, medication review, and clinical documentation to support better health outcomes.

Schedule:
- Flexible, day shift hours required for patient outreach and counseling
- Approximately 10-20 hours per week
- Guaranteed minimum of 8 paid hours weekly regardless of workload
Responsibilities:
- Conduct MTM consultations and provide medication education to patients
- Review medication profiles to identify optimization opportunities
- Accurately document clinical interventions and patient encounters
- Collaborate with program stakeholders to support patient outcomes
- Maintain compliance with state and federal regulations
Qualifications:
- Active and unrestricted California Pharmacist license
- Current California resident
- Previous MTM experience required
- Enrollment in California Medicaid (Medi-Cal) as an ORP (Ordering, Referring, and Prescribing) Provider is mandatory
- Strong communication and patient counseling skills
- Ability to work independently in a remote environment with schedule flexibility
