Pharmacist | Long Beach, California

A California-licensed Pharmacist is needed to support a remote Medication Therapy Management (MTM) pilot program designed to enhance clinical pharmacy initiatives. This part-time role offers flexible scheduling with 10-20 hours per week, working remotely from anywhere within California. The ideal candidate will have a strong background in MTM services, patient counseling, and clinical documentation, and must currently reside in California.

Responsibilities:

• Conduct comprehensive Medication Therapy Management consultations
• Provide patient counseling and medication education to improve adherence and outcomes
• Review medication profiles to identify opportunities for optimization and intervention
• Accurately document clinical interventions and patient encounters
• Collaborate with program stakeholders to promote positive patient outcomes
• Maintain compliance with all applicable state and federal regulations

Qualifications:

• Active and unrestricted California Pharmacist license
• Current residency in California is required
• Previous experience in Medication Therapy Management is essential
• Must be enrolled in California Medicaid (Medi-Cal) as an ORP (Ordering, Referring, and Prescribing) Provider
• Excellent communication and patient counseling skills
• Ability to work independently and manage flexible hours in a remote environment

Schedule:

• Flexible hours with a day-shift focus for patient outreach and counseling
• Approximately 10-20 hours per week, with a guaranteed minimum of 8 paid hours weekly regardless of workload
