Pharmacist in Long Beach, CA

A California-licensed Pharmacist is needed to support a remote Medication Therapy Management (MTM) pilot program. This part-time contract role offers the flexibility to work 10-20 hours per week from anywhere within California and requires residency in the state. The position focuses on delivering MTM services, patient counseling, and assisting in launching a clinical pharmacy initiative aimed at optimizing patient outcomes.

Schedule:

• Flexible hours, approximately 10-20 hours per week
• Day shift availability required for patient outreach and counseling
• Guaranteed minimum of 8 paid hours weekly regardless of workload

Key Responsibilities:

• Conduct comprehensive MTM consultations with patients
• Provide medication education and counseling to support adherence and safety
• Review medication profiles to identify and recommend optimization opportunities
• Accurately document clinical interventions and patient encounters
• Collaborate with program stakeholders to enhance patient care outcomes
• Ensure compliance with all relevant state and federal regulations

Required Qualifications:

• Active, unrestricted California Pharmacist license
• Residency within California is mandatory
• Previous experience delivering MTM services
• Enrollment in California Medicaid (Medi-Cal) as an ORP (Ordering, Referring, and Prescribing) provider is required
• Strong communication skills and ability to counsel patients effectively
• Self-motivated with the ability to work independently in a remote setting and adapt hours as needed

This role is part of a pilot program with evolving workflows and offers the chance to engage deeply in a clinical pharmacy initiative. Candidates will have the opportunity during the interview to explore daily responsibilities, operational details, and technology needs.

This is a 3-6 month contract with potential for extension based on pilot progress. Immediate start is preferred.
