A healthcare provider in Van Nuys, CA is seeking a full-time Pharmacist dedicated to delivering high-quality, patient-centered care with a long-term commitment. This temp-to-perm opportunity offers a chance to join a bilingual, collaborative team focused on clinical excellence and meaningful patient outcomes, particularly in diabetes management.

Location: • Van Nuys, CA

Schedule: • Monday to Friday, 9:00am–6:00pm • Rotating Saturdays every third week, 9:00am–1:00pm

Responsibilities: • Dispense approximately 250 prescriptions daily with precision, ensuring safety and compliance • Conduct comprehensive clinical medication reviews emphasizing appropriateness, interactions, dosing, and therapeutic outcomes • Provide detailed patient counseling, particularly on diabetes management including insulin use, injectables, continuous glucose monitors, oral agents, and lifestyle modifications • Monitor chronic disease states and perform clinical assessments within a predominantly diabetic patient population • Collaborate actively with prescribers to clarify orders, suggest therapy adjustments, and optimize clinical outcomes • Identify and address medication-related problems and adherence barriers • Oversee pharmacy workflow ensuring regulatory compliance • Foster a patient-focused environment promoting long-term health and quality care

Qualifications: • Bachelor of Science (B.S.) or Doctor of Pharmacy (PharmD) from an accredited institution • Active, clear California Pharmacist License • Minimum 2 years clinical pharmacy experience preferred, with diabetic clinical assessment experience highly valued • Strong clinical knowledge with confidence in diabetes management and insulin counseling • Bilingual in Spanish and English strongly preferred • Dependable, accountable, professional, and committed to long-term growth and excellence
