Schedule:
- Flexible scheduling with day shift hours for patient outreach and counseling
- Approximately 10-20 hours per week
- Guaranteed minimum of 8 paid hours weekly, regardless of workload
Responsibilities:
- Conduct MTM consultations to optimize medication regimens
- Provide patient counseling and medication education tailored to individual needs
- Review medication profiles and identify opportunities to enhance therapy outcomes
- Accurately document clinical interventions and patient interactions
- Collaborate with program stakeholders to improve patient health outcomes
- Ensure compliance with all applicable state and federal regulations
Qualifications:
- Active, unrestricted California Pharmacist license
- Must be a California resident
- Prior experience in Medication Therapy Management required
- Enrollment as a California Medicaid Medi-Cal ORP (Ordering, Referring, and Prescribing) provider is mandatory
- Strong communication skills with an emphasis on patient counseling
- Ability to work independently in a remote setting with flexible availability
