A California-licensed pharmacist resident is needed for a part-time, remote Medication Therapy Management (MTM) pilot program. This opportunity offers flexible scheduling, approximately 10-20 hours per week, with a guaranteed minimum of 8 paid hours weekly. The role focuses on conducting MTM consultations, providing patient counseling, and supporting a clinical pharmacy initiative aimed at optimizing patient outcomes.

- Remote within California (CA resident required)
Schedule:
- Flexible hours, approximately 10-20 hours per week
- Day shift availability for patient outreach and counseling
- Guaranteed minimum 8 hours paid weekly regardless of workload
Responsibilities:
- Conduct comprehensive MTM consultations
- Educate and counsel patients on medication use
- Review medication profiles to identify optimization opportunities
- Document clinical interventions and patient encounters accurately
- Collaborate with program stakeholders to promote patient health outcomes
- Ensure compliance with all state and federal regulations
Qualifications:
- Active and unrestricted California Pharmacist license
- Residency within California is mandatory
- Prior experience in MTM services required
- Enrollment in California Medicaid (Medi-Cal) as an Ordering, Referring, and Prescribing (ORP) provider is essential
- Strong communication skills and patient counseling expertise
- Self-motivated with the ability to work independently in a remote setting
