Pharmacist in Los Angeles, CA

A California-licensed Pharmacist is needed for a remote Medication Therapy Management (MTM) pilot program. This part-time contract role offers flexible scheduling (10-20 hours per week) and allows you to work remotely from anywhere within California. The engagement is expected to last 3-6 months with potential for extension based on pilot program progress. Candidates must currently reside in California and be enrolled in Medicaid as an Ordering, Referring, and Prescribing (ORP) provider.

Location:

• Remote (California residents only)

Schedule:

• Flexible scheduling with day shift hours for patient outreach and counseling
• Approximately 10-20 hours per week
• Guaranteed minimum of 8 paid hours weekly regardless of workload

Responsibilities:

• Conduct comprehensive MTM consultations with patients
• Provide patient counseling and medication education to optimize medication use
• Review medication profiles for opportunities to improve therapy and outcomes
• Document clinical interventions and encounters accurately and timely
• Collaborate with program stakeholders to enhance patient care and support clinical goals
• Ensure compliance with all relevant state and federal regulations throughout service delivery

Required Qualifications:

• Active, unrestricted California Pharmacist license
• Residency within California is mandatory
• Proven experience delivering Medication Therapy Management services
• Enrollment in California Medicaid (Medi-Cal) as an ORP provider is required
• Strong communication and patient counseling skills
• Ability to independently manage remote work and flexible hours effectively
