Schedule:

• Monday through Friday
• Day shift hours

Key Responsibilities:

• Accurately dispense medications and verify prescription orders.
• Collaborate with healthcare teams to develop and adjust medication therapy plans especially tailored to LTAC patient needs.
• Monitor patient responses to prescribed medications and manage any adverse effects.
• Maintain comprehensive records of medications and patient information.
• Ensure compliance with all regulatory standards and pharmacy laws.

Qualifications:

• Licensed Pharmacist with active credentials.
• Experience in a hospital or LTAC pharmacy preferred.
• Strong understanding of pharmacotherapy and medication management in long-term care settings.
• Effective communication skills for working with healthcare professionals and patients.
• Detail-oriented with a commitment to patient safety.
