An advanced pharmacy practice position is available for a Hematology/Oncology Pharmacist specializing in CAR-T cell therapy and Bone Marrow Transplant within a leading pediatric inpatient hospital setting in Madera, CA. This role involves delivering comprehensive pharmaceutical care to patients from neonates through young adulthood, emphasizing safe and effective drug therapy management in a highly specialized pediatric environment.

The direct hire position requires a Doctor of Pharmacy degree and active California pharmacist licensure. Candidates with completion of an ASHP-accredited residency, preferably with PGY2 specialization in oncology and pediatric pharmacy experience, will be highly regarded. Board Certification in Oncology Pharmacy (BCOP) and familiarity with FACT accreditation standards for hematopoietic cell transplant programs are preferred.

This role demands strong clinical expertise in oncology pharmacotherapy, particularly in hematopoietic cell transplant including CAR-T and Bone Marrow Transplant therapies. Responsibilities include reviewing physician orders, providing pharmacology consultation, advocating for patients, fostering medication safety, and collaborating closely with multidisciplinary healthcare teams. The pharmacist will also participate in staff development and training pharmacy students and residents.

Schedule:

  • 10-hour mixed shifts, primarily daytime and some weekends with limited evenings and overnight coverage to support 24/7 inpatient pharmacy operations.

Key Qualifications and Experience:

  • Doctorate in Pharmacy (PharmD) with active CA pharmacist license
  • PALS and BLS certifications (required), ACLS encouraged
  • Minimum two years pediatric oncology pharmacy experience preferred
  • PGY-2 Oncology residency and BCOP certification advantageous
  • EPIC EMR proficiency
