A rewarding opportunity is available for a Pharmacist to support patient care in a Long-Term Acute Care (LTAC) setting. This role involves working in a small hospital environment where your expertise will directly impact the health and recovery of patients requiring extended specialized care.

Location:

Schedule:

• Monday through Friday
• Day shift

Key Responsibilities:

• Accurately dispense medications, ensuring compliance with all regulations and hospital policies
• Collaborate with healthcare professionals to review and optimize medication therapy plans
• Provide patient counseling and support to promote safe and effective medication use
• Monitor inventory and manage pharmaceutical supplies for the LTAC facility
• Participate in medication reconciliation and clinical rounds as needed

Desired Experience:

• Licensed Pharmacist with experience in hospital or LTAC pharmacy settings preferred
• Strong knowledge of medication therapies used in long-term acute care
• Detail-oriented with excellent communication and problem-solving abilities
• Ability to work independently while being a proactive member of the healthcare team

Contract Details:

• Immediate start available
• Duration: 13 weeks or longer
