Home Infusion Pharmacist | Manteca, California

A part-time pharmacist opportunity is available to provide specialized home infusion pharmacy support, working approximately 2-3 days per week, Monday through Friday from 8:30 AM to 5:00 PM. This role offers ongoing contract work and is ideal for candidates with a passion for patient-centered care in infusion therapy, including recent graduates with relevant IV/TPN experience who are eager to develop their skills further.

Location:
Position is based in Manteca, CA. Candidates commuting from nearby areas such as Modesto, Stockton, or Merced are encouraged to apply.

Qualifications / Desired Experience:

• Valid California pharmacist license in good standing
• Strong background or interest in IV/Infusion and clinical pharmacy practices
• Preferred: Prior home infusion pharmacy experience or equivalent clinical training during internships or APPEs
• Ability to work effectively as a communicator and team player, emphasizing attitude and professionalism
• Attention to detail, especially in maintaining accurate and thorough documentation
• Recent graduates with IV/TPN experience are welcome as training will be provided by leadership

Key Responsibilities:

• Review and evaluate physician prescription orders for dosage appropriateness, drug interactions, and administration routes
• Maintain complete patient records including medical history, lab results, and medication profiles
• Ensure compliance with federal and state regulations as well as accrediting agency standards
• Coordinate with healthcare staff to order and assess laboratory tests for medication safety and efficacy
• Provide clinical and technical support to physicians, nurses, and patients
• Oversee sterile compounding and quality assurance programs
• Assist in policy implementation, training, and supervision of pharmacy personnel
• Document all professional communications in patient medical records
• Educate patients and caregivers on drug therapies and safety protocols
