Home Infusion Pharmacist - Modesto, CA

A part-time infusion pharmacist position is open at a home infusion pharmacy located in the Modesto area, offering 2-3 workdays per week, Monday through Friday, from 8:30 a.m. to 5:00 p.m. This contract role allows for ongoing employment starting immediately and is ideal for candidates based in Modesto, Stockton, Merced, or nearby communities.

This role is designed for pharmacists with solid IV/infusion experience, particularly those familiar with home infusion pharmacy settings. Recent graduates with IV/TPN experience gained during APPE rotations or internships are encouraged to apply, as training is available under the guidance of an experienced director who values strong communication skills and teamwork above all.

Key Responsibilities:

• Review and evaluate physician prescription orders for accuracy, dosage, appropriate drug choice, patient diagnosis, and potential interactions.
• Maintain comprehensive patient records, including medical history, current medications, lab results, and nutritional information.
• Assist with regulatory compliance documentation for state, federal, and accrediting organizations.
• Collaborate with healthcare professionals to ensure laboratory test results inform medication safety and effectiveness.
• Support physicians, nurses, and patients with clinical and technical guidance.
• Supervise sterile compounding and quality assurance programs.
• Train and oversee pharmacy technicians, interns, and other pharmacy staff.
• Document communications in patient medical records.
• Lead initiatives to maintain patient and staff safety and provide drug therapy education.

Qualifications:

• Current California pharmacist license in good standing.
• Strong background in IV therapy, infusion, and clinical pharmacy practices.
• Experience with home infusion pharmacy preferred.
• Proficiency with CareTend system is a plus.
• Candidates must submit an updated resume, skills checklist, one pharmacy manager reference, and a copy of their CA pharmacist license.
