Join a dynamic healthcare team as a Travel Pharmacist in Vermont, supporting a 100-bed community hospital affiliated with a major university health network. This role offers a unique opportunity to provide expert pharmaceutical care while gaining experience in a well-respected facility.

Location:

  • Berlin, Vermont

Duration:

  • 4 to 6 months, with an ASAP start date

Schedule:

  • Monday through Friday
  • Shifts available: 6:30 AM to 3:00 PM, 10:00 AM to 6:30 PM, or 12:00 PM to 8:00 PM

Qualifications and Experience:

  • Minimum of 5 years pharmacy experience
  • Strong intravenous (IV) medication expertise
  • Vermont Provisional Pharmacy License preferred, or eligibility to obtain a temporary license initially (with a commitment to acquire a permanent license during assignment)
  • Experience with medication reconciliation, order verification, and clinical dosing management

Key Responsibilities:

  • Perform accurate medication reconciliation for patient care continuity
  • Verify medication orders ensuring safety and compliance
  • Manage dosing and laboratory monitoring of vancomycin therapy
  • Oversee renal dosing adjustments to optimize therapeutic outcomes

Additional Information: Vermont offers a provisional license to pharmacists who meet qualifications, enabling you to start promptly while working towards permanent licensure. This assignment requires candidates willing to operate under a temporary license with plans to transition to permanent licensure.
