Duration:
- 4 to 6 months, with an ASAP start date
Schedule:
- Monday through Friday
- Shifts available: 6:30 AM to 3:00 PM, 10:00 AM to 6:30 PM, or 12:00 PM to 8:00 PM
Qualifications and Experience:
- Minimum of 5 years pharmacy experience
- Strong intravenous (IV) medication expertise
- Vermont Provisional Pharmacy License preferred, or eligibility to obtain a temporary license initially (with a commitment to acquire a permanent license during assignment)
- Experience with medication reconciliation, order verification, and clinical dosing management
Key Responsibilities:
- Perform accurate medication reconciliation for patient care continuity
- Verify medication orders ensuring safety and compliance
- Manage dosing and laboratory monitoring of vancomycin therapy
- Oversee renal dosing adjustments to optimize therapeutic outcomes
Additional Information: Vermont offers a provisional license to pharmacists who meet qualifications, enabling you to start promptly while working towards permanent licensure. This assignment requires candidates willing to operate under a temporary license with plans to transition to permanent licensure.
