Travel Pharmacist | Montpelier, Vermont

This opportunity offers a rewarding role as a Pharmacist within a community hospital setting in Vermont. The position is based in a 100-bed hospital that is part of a reputable health network. The role is travel-based, with an immediate start and a duration of approximately 4 to 6 months.

Schedule/Hours:

• Monday through Friday
• Shift options: 6:30 AM – 3:00 PM, 10:00 AM – 6:30 PM, or 12:00 PM – 8:00 PM

Key Responsibilities:

• Perform medication reconciliation for admitted patients
• Verify medication orders with accuracy
• Manage vancomycin dosing and associated laboratory monitoring
• Adjust medication dosages based on renal function
• Support clinical pharmacy operations in line with hospital protocols

Qualifications and Experience:

• Minimum of 5 years of pharmacy experience, preferably with strong intravenous (IV) medication expertise
• Licensed pharmacist or ability to obtain a Vermont provisional (temp) license promptly, followed by a permanent license
• Willingness to work under a temporary license while pursuing permanent licensure
• Ability to deliver precise, patient-centered pharmaceutical care in a fast-paced hospital environment
