Exciting opportunity for two Oncology/Hematology inpatient pharmacists to join a respected medical center known for advancing patient care. Schedule: Typical inpatient schedule with day and evening shifts, rotating weekends

Key Responsibilities:
- Participate in daily interdisciplinary rounds to design, implement, and monitor medication regimens for oncology and SCT patients
- Review and verify chemotherapy and other medication orders, ensuring proper dosing, form, interaction checks, and cost-effectiveness
- Reconcile home medications upon patient admission
- Communicate medication clarifications with prescribers and document all changes
- Counsel patients and families on medication regimens, adherence, and adverse effects prior to discharge
- Provide pharmacokinetic consultations, including dose adjustments for vancomycin or aminoglycosides
- Detect, monitor, and report adverse drug reactions and medication errors
- Support nursing staff with medication-related questions
- Facilitate chemotherapy desensitizations as needed
- Perform order verification and manage in-basket messages for assigned units
Desired Qualifications:
- Bachelor of Science in Pharmacy
- Illinois pharmacist license or eligibility to obtain one
- Postgraduate ASHP PGY2 Hematology/Oncology residency highly preferred
- Board Certification in Oncology Pharmacy (BCOP) preferred within 18 months of hire
- BLS certification
- Expertise in intravenous admixture, drug therapy, chemotherapy, and cellular therapies
- Strong problem-solving skills and ability to work independently in a fast-paced environment
- Will consider candidates with solid oncology pharmacy experience, even if residency-trained candidates are preferred
