Key Responsibilities:

• Review all medication orders from physicians and physician extenders, verifying appropriateness of drug choice, dosage, frequency, and route.
• Assess patient allergies, medical history, drug interactions, and lab results to avoid clinical conflicts before medication dispensing.
• Manage inventory control to maintain adequate medication supply.
• Provide accurate drug information and support to healthcare team members.
• Supervise pharmacy students and technicians during medication preparation, ensuring safety and accuracy.
• Collaborate with the healthcare team for continuum of care planning and discharge readiness.

Qualifications and Experience:

• Bachelor’s Degree from a College of Pharmacy accredited by the American Council on Pharmaceutical Education is required; Doctor of Pharmacy (PharmD) preferred.
• Minimum of 1 year hospital experience or completion of Advanced Pharmacy Practice Experience (APPE) in a clinical environment is preferred.
• Current PA Pharmacist License is mandatory.
