Responsibilities
- Provide comprehensive pharmaceutical care to patients within an LTAC setting.
- Collaborate closely with the multidisciplinary healthcare team to optimize medication therapy.
- Utilize Wellsky and PYXIS systems efficiently for medication management and automation.
- Monitor pharmacokinetics, anticoagulation protocols, and TPN therapies as applicable.
- Ensure compliance with hospital policies, regulatory standards, and patient safety protocols.
Desired Qualifications & Experience
- Bachelor of Science Degree in Pharmacy required; Doctor of Pharmacy (PharmD) preferred.
- Active, valid California Pharmacist License required.
- 2-3 years of retail pharmacy experience preferred for clinicians looking to maintain clinical expertise; acute care or inpatient hospital experience is highly valuable.
- Experience with pharmacokinetics, anticoagulation, and TPN preferred.
- Current BLS and ACLS certification from an approved vendor preferred.
- Candidates with 10+ years exclusively in community pharmacy are not preferred; this role suits professionals eager to engage in clinical settings.
- PGY1 residency candidates may find the smaller facility size less desirable for long-term placement.
