Pharmacist Opportunity – LTAC Setting (Part-Time, Days) – Kentfield, CA
This role offers the chance to work in a Long-Term Acute Care (LTAC) facility with 50-60 beds, ideal for pharmacists seeking to apply their clinical skills in a focused inpatient setting. The position is part-time with the potential to transition to full-time, primarily covering day shifts. Familiarity with Wellsky and PYXIS automation systems is essential.

Key Responsibilities:
- Provide clinical pharmacy services in an LTAC environment
- Manage pharmacokinetics, anticoagulation, and TPN therapy
- Collaborate with healthcare teams to optimize patient care and medication use
- Utilize Wellsky and PYXIS systems for medication distribution and management
Qualifications and Experience:
- Bachelor of Science Degree from an accredited College/School of Pharmacy (required)
- CA Pharmacist License (required)
- Doctor of Pharmacy degree preferred
- 2-3 years of retail pharmacy experience welcomed, especially for those wanting to maintain clinical knowledge in a hospital setting
- Inpatient or hospital experience preferred; candidates with over 10 years in community pharmacy are not preferred
- Experience with pharmacokinetics, anticoagulation, and TPN strongly preferred
- Current BLS and ACLS certifications from an approved vendor preferred
- Prior acute care experience is advantageous
