Home Infusion Pharmacist in Oakdale, CA

A well-established home infusion pharmacy is seeking a part-time infusion pharmacist to provide expert support 2-3 days per week. This role offers the opportunity to work Monday through Friday from 8:30 a.m. to 5:00 p.m. and is ideal for candidates located near Oakdale, CA or within a commutable distance such as Modesto, Stockton, or Merced. The position is contract-based with an immediate start and ongoing availability.

This role focuses on home infusion services, requiring a pharmacist who excels in clinical evaluation and sterile compounding. The director values strong teamwork and communication skills and is open to training candidates who have hospital IV/TPN experience or recent graduates with relevant internship or APPE exposure.

Key Responsibilities:

• Review and evaluate physician prescription orders for dosage appropriateness, drug choice, and potential interactions.
• Verify and maintain patient data including medical history, lab results, and concurrent medications.
• Manage documentation compliance with state, federal, and accreditation standards.
• Coordinate and interpret clinical laboratory tests to ensure medication safety and efficacy.
• Provide clinical and technical support to healthcare professionals and patients.
• Supervise sterile compounding processes and quality assurance programs.
• Train and oversee pharmacy staff including technicians and interns.
• Document professional communications within patient medical records.
• Educate patients and caregivers on drug therapy instructions.

Qualifications:

• Active California pharmacist license in good standing.
• Strong IV/infusion and clinical pharmacy experience, preferably in home infusion settings.
• Experience with CareTend system is preferred.
• Excellent communication skills and a collaborative mindset.
• Updated, detailed resume with clear month/year entries and duties.
• One pharmacy manager reference required.
