Pharmacist - Pasadena, California

This opportunity is ideal for pharmacists interested in transitioning patients safely from hospital to home with a focus on Transitions of Care and Ambulatory Care services. Preferred candidates are recent PGY1 residents, but those without postgraduate residency training who have retail or outpatient pharmacy experience will also be considered. Fluency in Spanish is a valuable asset for enhancing communication and patient engagement.

Qualifications and Desired Experience:

• Licensed Pharmacist with a valid California license
• Recent PGY1 residency preferred; open to non-PGY1 pharmacists with retail or outpatient pharmacy backgrounds

• Experience or strong interest in ambulatory and transitions of care pharmacy
