Pharmacist - Portland, Oregon

A remote opportunity is available for a Clinical Pharmacist Specialist (CPS) to provide expert pharmacy services focused on managed care populations. This role supports high-quality, evidence-based medication management and cost-effective care coordination for health plan members. The CPS collaborates closely with medical directors, nurse case managers, and network providers to ensure optimal patient outcomes.

Key Responsibilities:

• Deliver direct patient care through Medication Therapy Management (MTM) programs.
• Manage population-based disease initiatives and specialty pharmaceutical acquisition.
• Conduct drug utilization reviews and develop clinical pharmacy programs.
• Participate in clinical policy formulation, including Pharmacy and Therapeutics Committee duties.
• Review pharmacy and medical claims, interact with members and caregivers, and develop medication-related problem lists.
• Communicate therapeutic recommendations to physicians clearly and professionally to implement necessary changes.
• Provide education and consultation to healthcare team members, ensuring alignment with best practices.
• Maintain professional development through ongoing education and adapt to a fast-paced, evolving healthcare environment.
• Enhance quality of care and optimize health outcomes while effectively managing costs.

Desired Qualifications:

• Active Pharmacist license from Oregon, Washington, or California.
• Minimum of 3 years’ experience in a managed care environment, including direct involvement with prior authorizations, audits, appeals, and ALJ court hearings.
• Strong communication skills, with the ability to influence physician prescribing habits constructively.
• Proven ability to prioritize responsibilities and manage workload effectively.
• Commitment to continuous professional growth and adapting to healthcare changes.

Schedule:

• Monday through Friday, remote work.
• Rare weekend coverage required a few times per year.
