Join a healthcare setting specializing in Long-Term Acute Care (LTAC) as a Staff Pharmacist dedicated to enhancing patient outcomes through expert medication management. This 13-week contract opportunity requires an immediate start and offers a dynamic environment where clinical expertise and communication skills are paramount.

Key Responsibilities:

  • Perform medication reconciliation focusing on accurate discharge medication lists.
  • Collaborate with patients, families, and pharmacies to gather comprehensive home medication histories prior to admission.
  • Provide discharge counseling to ensure patient and family understanding of medication regimens.
  • Apply clinical knowledge in antimicrobial stewardship by interpreting culture results and making informed recommendations for conditions such as urinary tract infections, pneumonia, and skin and soft tissue infections.
  • Manage pharmacokinetic dosing, including anticoagulation therapy, with ongoing monitoring and adjustments.
  • Utilize EPIC electronic health records to document and support clinical decisions.

Qualifications and Desired Experience:

  • Previous hospital or LTAC pharmacy experience is essential.
  • Proficient in EPIC systems.
  • Strong clinical understanding of antimicrobial stewardship and pharmacokinetic dosing.
  • Effective communication skills to facilitate collaboration with multidisciplinary teams and patients.
  • Current Basic Life Support (BLS) certification.
