A pharmacist position focused on sterile compounding is available for a 13-week contract in San Antonio, TX. The schedule for this role is 12 PM to 8 PM. This position emphasizes home infusion therapy and medication safety within a clinical setting.

Responsibilities include evaluating patients to determine eligibility for home infusion therapy and developing individualized medication therapy plans based on clinical assessments, lab values, and pharmacokinetics. The pharmacist will prepare, verify, and dispense sterile medications according to USP standards and regulatory requirements. Educating patients, caregivers, and healthcare providers on proper medication use and infusion administration is essential. Collaboration with healthcare teams to ensure continuity of care and monitoring of patient outcomes will be a key function. Additionally, managing infusion pumps, maintaining quality assurance, providing drug information, and supporting regulatory compliance are part of daily duties.

Qualifications for this role require a Doctor of Pharmacy (PharmD) degree or graduation from an accredited School of Pharmacy, along with an active, unrestricted Texas Pharmacist license. Candidates should have at least one year of sterile IV compounding experience, or alternatively, experience in non-sterile compounding, immunizations, Medication Therapy Management (MTM), completion of an ASHP-accredited residency, completion of an Advanced Pharmacy Practice Experience (APPE), or equivalent pharmacy experience.

Strong clinical knowledge of home infusion pharmacy, ability to interpret laboratory values, and apply pharmacokinetic principles are essential. Excellent communication, patient counseling, and customer service skills are also required.
