Pharmacist - San Diego, California

A California-licensed Pharmacist is needed for a remote Medication Therapy Management (MTM) pilot program. This part-time contract position offers flexible scheduling with approximately 10-20 hours per week, allowing you to work from anywhere within California. The contract term is 3 to 6 months, with potential for extension depending on the program's duration.

Location:

• Remote (California residents only)

Schedule:

• Flexible hours, primarily daytime to support patient outreach and counseling
• Guaranteed minimum of 8 paid hours per week regardless of workload

Responsibilities:

• Conduct MTM consultations to optimize medication use and improve patient outcomes
• Provide patient education and medication counseling with strong communication skills
• Review medication profiles and identify clinical intervention opportunities
• Document clinical interventions and encounters with accuracy and attention to detail
• Collaborate with program stakeholders to support clinical pharmacy initiatives
• Ensure compliance with applicable state and federal pharmacy regulations

Qualifications:

• Active, unrestricted Pharmacist license in California
• Current residency within California is required
• Previous experience performing MTM services
• Enrollment as an Ordering, Referring, and Prescribing (ORP) Provider in California Medicaid (Medi-Cal) is mandatory
• Ability to work independently in a remote setting and manage a flexible schedule
