Pharmacist - San Diego, California

A California-licensed Pharmacist is needed to join an innovative remote Medication Therapy Management (MTM) pilot program. This part-time role offers flexible scheduling with approximately 10-20 hours per week, allowing you to work remotely from anywhere within California. The position focuses on supporting clinical pharmacy services through patient counseling, medication reviews, and collaboration with program stakeholders to optimize patient outcomes.

Key Responsibilities:

• Conduct comprehensive MTM consultations with patients
• Provide thorough patient counseling and medication education
• Review medication profiles to identify and recommend therapy optimizations
• Document clinical interventions and patient encounters accurately and timely
• Collaborate with program stakeholders to enhance patient care and program success
• Ensure compliance with all relevant state and federal pharmacy regulations

Qualifications:

• Active, unrestricted California Pharmacist license
• Residency within California is mandatory
• Prior experience delivering Medication Therapy Management services required
• Current enrollment in California Medicaid (Medi-Cal) as an Ordering, Referring, and Prescribing (ORP) Provider is required
• Strong communication skills enabling effective patient counseling
• Ability to work independently in a remote environment with flexible scheduling availability during day shift hours

Schedule:

• Flexible hours, approximately 10-20 per week
• Guaranteed minimum of 8 paid hours weekly regardless of workload
• Day shift hours required for patient outreach and counseling
