A California-licensed Pharmacist is needed for a remote Medication Therapy Management (MTM) pilot program. This part-time contract position offers flexible scheduling with approximately 10-20 hours per week, allowing you to work from anywhere within California. The contract term is 3 to 6 months, with potential for extension depending on the program's duration.
Location:
- Remote (California residents only)
Schedule:
- Flexible hours, primarily daytime to support patient outreach and counseling
- Guaranteed minimum of 8 paid hours per week regardless of workload
Responsibilities:
- Conduct MTM consultations to optimize medication use and improve patient outcomes
- Provide patient education and medication counseling with strong communication skills
- Review medication profiles and identify clinical intervention opportunities
- Document clinical interventions and encounters with accuracy and attention to detail
- Collaborate with program stakeholders to support clinical pharmacy initiatives
- Ensure compliance with applicable state and federal pharmacy regulations
Qualifications:
- Active, unrestricted Pharmacist license in California
- Current residency within California is required
- Previous experience performing MTM services
- Enrollment as an Ordering, Referring, and Prescribing (ORP) Provider in California Medicaid (Medi-Cal) is mandatory
- Ability to work independently in a remote setting and manage a flexible schedule
