Pharmacist - San Fernando, California

Provide critical medication support as a pharmacist specializing in Transitions of Care and Ambulatory Care at a major 400-bed hospital in the San Fernando Valley region. This role focuses on ensuring patients receive proper medication education and counseling before discharge, helping to improve medication adherence and promote positive health outcomes through a ‘meds to beds’ program.

Key Responsibilities:

• Collaborate with patients prior to discharge to educate them about their medication regimens and address any concerns.
• Facilitate medication refills before patients leave the hospital, enhancing compliance and reducing readmissions.
• Work closely with healthcare teams to support seamless transitions from hospital to home care.
• Support ambulatory care initiatives within the hospital and outpatient clinics.

Qualifications:

• Licensed pharmacist with strong clinical knowledge.
• Recent PGY1 residency graduates are preferred; non-PGY1 candidates will also be considered.
• Experience or interest in transitions of care, medication reconciliation, and patient counseling is valuable.
