A rewarding opportunity is available for a Pharmacist specializing in Transitions of Care/Ambulatory Care within a 400-bed hospital in the San Fernando Valley region. This role focuses on supporting patients at discharge by providing medication education, counseling, and assistance with prescription refills to promote medication adherence and improve health outcomes. This program, often referred to as a 'meds to beds' initiative, is essential in ensuring patients maintain their medication regimens once they leave the hospital.

Key Responsibilities:

  • Engage with patients prior to discharge to educate and counsel on medications
  • Assist patients in obtaining medication refills before leaving the hospital
  • Collaborate with healthcare teams to support the discharge process and ensure continuity of care
  • Contribute to programs designed to reduce hospital readmissions by improving medication adherence

Qualifications and Desired Experience:

  • Licensed Pharmacist with strong communication skills
  • Focused interest or experience in Transitions of Care or Ambulatory Care preferred
  • New Pharmacy Graduates (PGY-1) are highly encouraged to apply; candidates without PGY-1 experience will also be considered
  • Ability to work in a fast-paced hospital environment
  • Commitment to patient-centered care and improving health outcomes

  • This position is a long-term contract opportunity within the Los Angeles County Department of Health System, a large public health system that includes outpatient clinics and multiple hospitals.
  • Pharmacists are hired through an agency but have the option to apply to become permanent county employees in the future.
