Pharmacist - San Francisco, California

Pharmacist opportunity in a Long-Term Acute Care (LTAC) setting, supporting a 50-60 bed facility located in Kentfield, CA. This role offers a part-time schedule with the potential to increase to full-time hours, working day shifts. The position utilizes Wellsky and PYXIS automation systems.

Key Responsibilities:

• Provide comprehensive pharmaceutical care tailored to long-term acute care patients.
• Manage medication distribution and ensure accurate dispensing.
• Collaborate with healthcare teams to support clinical interventions, including pharmacokinetics and anticoagulation management.
• Utilize clinical knowledge to optimize patient outcomes in an inpatient setting.
• Maintain compliance with state and federal pharmacy regulations and facility policies.

Qualifications and Desired Experience:

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy degree strongly preferred.
• Valid California Pharmacist License.
• 2-3 years of retail pharmacy experience is considered, especially for those seeking to maintain or expand clinical skills in a long-term care environment.
• Preference for candidates with inpatient or hospital pharmacy experience.
• Experience with pharmacokinetics, anticoagulation, and Total Parenteral Nutrition (TPN) is highly valued.
• Current BLS and ACLS certifications preferred.
• Acute care pharmacy experience is an advantage.
