This opportunity is ideal for a Pharmacist interested in supporting patients in a Long-Term Acute Care (LTAC) environment, with 50-60 beds, located in Kentfield, CA. The position offers a part-time schedule with potential for full-time hours, working daytime shifts, providing clinical pharmacy services within a well-established system utilizing Wellsky and PYXIS automation.

Key Responsibilities:

  • Deliver comprehensive pharmaceutical care to LTAC patients.
  • Manage medication therapy in collaboration with interdisciplinary teams.
  • Utilize Wellsky and PYXIS systems for medication dispensing and management.
  • Apply clinical expertise in pharmacokinetics, anticoagulation, and TPN for acute patient care.
  • Maintain compliance with pharmacy regulations and ensure patient safety.

Qualifications and Desired Experience:

  • Bachelor of Science Degree from an accredited College/School of Pharmacy required.
  • California Pharmacist License is mandatory.
  • Doctor of Pharmacy (PharmD) degree preferred.
  • 2-3 years of retail pharmacy experience considered if seeking a clinical role.
  • Inpatient or hospital pharmacy experience strongly preferred.
  • Knowledge of pharmacokinetics, anticoagulation, and TPN clinically preferred.
  • Previous acute care pharmacy experience advantageous.
  • Current and active BLS and ACLS certifications from an approved vendor preferred.
  • Candidates with extensive community pharmacy experience (10+ years) may not be the best fit.
  • This role may not appeal to recent PGY1 graduates due to the facility’s smaller size.
