Pharmacist | San Rafael, California

This opportunity is for a Pharmacist in a Long-Term Acute Care (LTAC) setting with 50-60 beds, located in Kentfield, CA. The role offers part-time hours with the possibility to work full-time, primarily daytime shifts. The work environment utilizes Wellsky and PYXIS automation systems.

Key Responsibilities:

• Provide pharmaceutical care in the LTAC setting, including medication management and patient counseling.
• Utilize automation systems such as Wellsky and PYXIS effectively.
• Collaborate with clinical teams to support inpatient care.
• Monitor pharmacokinetics and anticoagulation therapies as applicable.
• Participate in TPN management and clinical protocols as needed.

Qualifications and Desired Experience:

• Bachelor of Science Degree from an accredited College or School of Pharmacy is required.
• Current active CA Pharmacist License.
• Doctor of Pharmacy degree preferred.
• 2-3 years of retail pharmacy experience welcomed, especially candidates seeking to maintain or enhance clinical skills in a hospital setting.
• Experience in inpatient/hospital pharmacy preferred.
• Familiarity with pharmacokinetics, anticoagulation, and TPN preferred.
• Current BLS and ACLS certifications preferred.
• Prior acute care experience is a plus.
• Candidates with extensive community pharmacy experience (10-12 years) are not preferred.
• This role might not be ideal for PGY1 residency graduates seeking long-term placement due to the facility size.
