Provide specialized clinical pharmacy services focused on infectious diseases, ensuring optimal antimicrobial use and patient outcomes. This direct hire role is based in Sterling Heights, MI, offering day shift hours with a focus on infectious diseases pharmacy practice.
Responsibilities:
- Deliver primary and consultative clinical pharmacy services in infectious diseases to patients and healthcare providers.
- Review and evaluate medication orders for appropriateness, efficacy, safety, and cost-effectiveness.
- Assess patient drug therapies, conducting pharmacokinetic analyses and monitoring therapeutic outcomes.
- Participate in patient care rounds with healthcare teams to recommend antimicrobial selections, dosing, and administration.
- Lead or support Antimicrobial Stewardship Programs, interpret microbiology data, and recommend IV-to-PO conversions.
- Manage therapeutic drug monitoring, including vancomycin and aminoglycosides.
- Educate and provide training to pharmacy staff, medical students, residents, and healthcare professionals.
- Collaborate with infectious diseases physicians, microbiology labs, infection prevention, and nursing staff.
- Develop clinical guidelines, order sets, and formulary restrictions related to infectious diseases.
- Engage in clinical research, educational program development, and participation in medication use evaluation activities.
Qualifications:
- Doctor of Pharmacy (PharmD) degree; PGY-1 residency completed, PGY-2 in Infectious Diseases training or BCIDP certification preferred.
- Registered Pharmacist with Controlled Substance licensure in Michigan (to be obtained within 3 months of hire).
- One to two years of clinical pharmacy experience in healthcare, ideally with infectious diseases focus.
- Strong background in microbiology, pharmacokinetics, and antimicrobial stewardship principles.
- Excellent communication and collaborative skills for interdisciplinary teamwork.
