Pharmacist Opportunity – Transitions of Care Focus

A 400-bed hospital in the San Fernando Valley region of Los Angeles is seeking a dedicated pharmacist to join their team on a contract basis. This role focuses on Transitions of Care and Ambulatory Care within a major county health system, providing critical medication support as patients transition from hospital to home.

Key Responsibilities:

• Provide medication education and counseling to patients prior to hospital discharge
• Facilitate medication refills before patients leave the hospital to ensure continuity of care
• Support a "meds to beds" program aimed at improving patient compliance with medication regimens post-discharge
• Collaborate with multidisciplinary healthcare teams to optimize patient outcomes across ambulatory care settings

Desired Experience:

• Licensed Pharmacist with experience or interest in Transitions of Care or Ambulatory Care pharmacy
• New PGY1 residency graduates are preferred but non-PGY1 pharmacists will also be considered
• Strong communication skills to effectively counsel patients on medication use and address questions
• Ability to work within a large health system serving diverse patient populations
