Home Infusion Pharmacist - Turlock, CA

A pharmacy opportunity is available for an infusion pharmacist to support home infusion services on a part-time basis (2-3 days per week) at a site near Turlock, CA. This role focuses on managing physician prescription orders, evaluating medication appropriateness, and ensuring quality care for patients receiving IV and TPN therapies. The position requires collaboration with healthcare teams and maintaining compliance with regulatory and quality standards.

Location:

• Near Turlock, CA, ideal for candidates commuting from Modesto, Stockton, or Merced

Work Schedule:

• Monday through Friday, 8:30 AM to 5:00 PM
• Part-time, 2-3 days per week

Key Responsibilities:

• Review and evaluate physician/prescriber orders for appropriate dosage, drug choice, administration route, and potential interactions
• Ensure patient records are complete, including medical history, laboratory results, and concurrent medications
• Maintain all required documentation to meet state, federal, and accreditation standards (e.g., JCAHO, ACHC)
• Oversee and participate in sterile compounding and quality assurance processes
• Provide clinical and technical support to physicians, nurses, and patients
• Assist in training and supervising pharmacy staff and interns
• Communicate medication therapy instructions to patients and caregivers
• Exercise best pharmacy practices to ensure patient and staff safety

Qualifications:

• Current California pharmacist license in good standing
• Strong IV, infusion, and clinical pharmacy experience; home infusion pharmacy experience highly preferred
• Experience with CareTend system is advantageous
• Excellent communication skills and a collaborative approach to teamwork
• Willingness to learn and grow; training available for candidates with relevant IV/TPN hospital experience or recent graduates with APPE/internship experience
