Support medication management and clinical pharmacy services in a Long-Term Acute Care (LTAC) facility with 50-60 beds, providing expert pharmaceutical care to a diverse patient population. Schedule/Hours:
- Day shift
- Part-time with opportunity to increase to full-time
Key Responsibilities:
- Provide comprehensive pharmaceutical care tailored to the needs of LTAC patients
- Monitor and adjust pharmacokinetic parameters and anticoagulation therapies
- Collaborate with healthcare teams to optimize inpatient medication management, including Total Parenteral Nutrition (TPN)
- Utilize Wellsky and PYXIS systems to support medication dispensing and automation workflows
- Ensure compliance with all regulatory and safety guidelines in pharmaceutical practice
Qualifications and Desired Experience:
- Bachelor of Science Degree from an accredited College or School of Pharmacy (Doctor of Pharmacy preferred)
- Current, valid California Pharmacist License
- 2-3 years of retail experience welcomed, especially candidates seeking to maintain and develop clinical expertise in a hospital environment
- Experience in inpatient or hospital pharmacy preferred, including pharmacokinetics, anticoagulation, and TPN
- Valid BLS and ACLS certifications strongly preferred
- Acute care experience is a significant plus
