A full-time clinical pharmacist position is available on a contract-to-hire basis, offering an opportunity to join a quality-focused team in Van Nuys, CA. This role suits a bilingual (Spanish & English) pharmacist committed to long-term growth and patient-centered care.
Location: Van Nuys, CA
Contract Terms: 13 weeks (Temp-to-Perm)
Schedule:
- Monday to Friday, 9:00 am – 6:00 pm
- Rotating every 3rd Saturday, 9:00 am – 1:00 pm
Key Responsibilities:
- Dispense approximately 250 prescriptions daily with accuracy and compliance
- Conduct thorough clinical medication reviews for appropriateness, dosing, interactions, and therapeutic outcomes
- Provide extensive patient counseling, emphasizing diabetes management including insulin use, injectables, CGMs, oral agents, and lifestyle advice
- Perform clinical assessments and monitor chronic diseases, focusing on a large diabetic patient population
- Collaborate with prescribers to clarify orders and recommend therapy adjustments
- Identify and resolve medication-related issues and barriers to adherence
- Oversee pharmacy workflow and ensure all regulatory requirements are met
- Foster a patient-focused environment aimed at long-term health outcomes
Desired Qualifications:
- B.S. or PharmD from an accredited pharmacy program
- Active and clear California Pharmacist License
- 2+ years of clinical pharmacy experience preferred
- Experience in diabetic clinical assessments is a strong plus
- Bilingual proficiency in Spanish and English strongly preferred
- Reliable, accountable, professional, with excellent clinical knowledge and judgment
- Confident in diabetes management and patient counseling
