A leading pharmacy is seeking a dedicated and bilingual Clinical Pharmacist (Spanish & English) to join their team in Van Nuys, CA. Location:

• Van Nuys, CA

Schedule:

• Monday to Friday, 9:00 AM to 6:00 PM
• Rotating every third Saturday, 9:00 AM to 1:00 PM

Key Responsibilities:

• Verify and dispense approximately 250 prescriptions daily with precision and attention to safety and compliance
• Conduct comprehensive clinical medication reviews to assess appropriateness, interactions, dosing, and therapeutic outcomes
• Provide extensive patient counseling, particularly focusing on diabetes management including insulin use, injectables, continuous glucose monitors (CGMs), and lifestyle guidance
• Perform clinical assessments and monitor chronic diseases, with a significant emphasis on diabetic patients
• Collaborate closely with prescribers to clarify orders, recommend therapy adjustments, and optimize patient care
• Identify and resolve medication-related issues and barriers to adherence
• Oversee pharmacy workflow and maintain regulatory compliance
• Foster a patient-centered approach that promotes long-term health and quality care

Qualifications:

• B.S. or PharmD from an accredited Pharmacy School
• Active, clear California Pharmacist License
• Minimum of 2 years clinical pharmacy experience preferred, especially with diabetic patient assessments
• Bilingual in Spanish and English strongly preferred
• Demonstrated reliability, professionalism, and dedication to quality over years of experience
